Training Provider Terms and Conditions
I hereby agree to the following terms and condition in relation to training provider as set in the provisions in Section 44 of the Employment Insurance System (Act 800):
(i)	Agree to provide Training approved by SOCSO, whereby place and time set.
(ii)	Will always abide all the terms and conditions of the agreement set by SOCSO;
(iii)	Will always provide right training, and be responsible in support of SOCSO's efforts and aspirations for human capital development;
(iv)	Will not commit and / or abet or engage in fraud, misappropriation, declaration of false information to benefit from SOCSO;
(v)	Will not abuse the information and / or any SOCSO title in the interest of himself or any of the parties without the written consent of the SOCSO;
(vi)	Will not behave in a manner that may aggravate or defame SOCSO's goodwill, integrity and reputation;
(vii)	Will promptly report to SOCSO if any attempt or action by any party may be construed as contrary to the provisions of the SIP 2017 Act or any relevant law.
(viii)	If convicted of violating the Statutory Declaration, may be liable to the Terms and Conditions pursuant to SOCSO training and the matters set out under the SIP 2017 Act, the MACC Act, 2009 or any relevant law.
